When you come in for your appointment, please bring the following:

• Insurance Cards/Information
  
• List of Current Medications
  
• X-Rays
  
• Any Other Records You Think are Important
  
• CoPays are due at time of service

There is a $35.00 charge for appointments not cancelled within 24 hours.
